The Perioral Triangle: Why Tretinoin Burns Here First

If you’re on tretinoin, you know the spot.

It’s not your cheeks. It’s not your forehead.

It’s the triangle, corners of your nose, nasolabial folds, around your mouth, chin.

That’s where it starts: the sting, the peeling, the raw feeling that makes you want to quit.

Why the perioral triangle fails first

  1. It’s anatomically thinner. Less barrier to begin with. Tretinoin penetrates deeper, faster.

  2. It moves constantly. Talking, eating, facial expression. Every microabrasion from tret reopens throughout the day.

  3. It’s exposed constantly. Drinking water, toothpaste, lip products, climate. TEWL is significantly higher than your cheeks.

  4. Your tretinoin migrates here. Even if you avoid applying it, product creeps into these folds.

This is the zone where most people tap out. Because “barrier repair cream” doesn’t cut it. It burns. It pills. It does nothing for the inflamed, broken feeling.

How I use it to stay on 0.05%

Night: Apply tretinoin everywhere except the triangle. Wait 30 min. Press a pea-size of Formula 32 into corners of nose, around mouth, chin. Nothing else.

Morning: If flaking, apply an ultra-thin layer to the same spots before SPF. The zinc oxide adds photoprotection exactly where you’re most photosensitive.

After week 6, my barrier caught up. Now I use Formula 32 2-3x/week as maintenance.

The point isn’t to “buffer your whole face.” It’s to rescue the zone that quits first.

If you can keep the perioral triangle calm, you can stay consistent with tret. If this zone flares, you’ll stop. I almost did.
